NEW SEMESTER SEVIS Immigration Registration Form

To complete the immigration registration for the new semester, you must complete the following form.

  • Please fill out this form completely and click on the Submit this Form button to send it to the International Student Office.
  • Be ready to provide a copy of a new visa and/or passport, if you have renewed the documents during the break.
  • If you’re Canadian and the requested information isn’t applicable, please enter “ N/A.”

All fields are required.
Dates should be entered as Month/Day/Year (e.g. 09/19/2003).
